ENDOSCOPIC TREATMENT OF FIBROEPITHELIAL POLYP OF THE URETER. Dario Garcia-Rojo, Mercedes Ramon, Natalia Garcia-Artero, Laura Moreno-Artero, Edmundo Tremps Hospital Sant Rafael. Barcelona. Spain. Introduction and Objectives: Ureteral fibroepithelial polyps are rare mesenchimal tumours. Due to the benign nature of the lesions, most authors recommend conservative surgery with minimally invasive techniques. Matherial and Methods: We present the case of a 72-year-old female with history of breast cancer, which came to te office reporting an irritative voiding simptoms. Utrasonograpy showed a pediculated neoformation of about 1.7 centimeters protruding into the bladder.Intravenous urography showed a filling deffec in the bladder with a low ectasia in pelvic ureter. Results: The vesical exploration was performed under spinal anaesthesia, showing a polypoid neoformation protruding through the right ureteral orifice, with a smooth surface. A right rigid ureteroscopy was perormed, which showed a neoformation, about 5 cm long, originated in the pelvic ureter, with a pediculated base of insertion and a paratumoral small ureteral stenosis. With the diagnostic suspicion of fibroepithelial ureteral polyp, we decided to section the pedicle and to perform a complete tumour extraction. The bed of the polip was biopsed whit a biopsy forceps, and a right double-j catheter was placed. The histopathological study confirmed a fibroepithelial poliyp. After ten years of follow-up, there were no findings of disease recurrence or ureteral stenosis. Conclusions:The fibroepithelial polyp is the most frequent benign ureteral tumour. It is important to establish a differential diagnosis with urothelial carcinoma of the upper urinary tract. In the event of suspicion, the most indicated approach is the ureteroscopic exploration and , if it is possible , to make a definitive treatment by means of endoscopic exeresis of the tumour.
