Here is the basic making of a google form to give multiple questions to an answer.

Here we use the fill handle to drag the formula. We then "paste special", using values only, the information. NOTE: This comes out as a string, in "quotation marks." To get rid of them, a quick hack is to open up an empty doc, then use find and replace the " with nothing, so we eliminate them.
